Your DEI Consultation Agreement
This Agreement explains your Dynamic Emotional Integration® Consultation. If you have any questions, please contact me by e-mail.
The Consultation Process
Our initial consultation will focus upon the issues you identify in the questionnaire on the next page. Any follow-up consultations will focus upon your progress, questions, or other issues that may arise.
I will send you a follow-up email within a week of your consultation, covering the topics we discussed and the practices and ideas we came up with, based on your needs.
Pricing and Cancellation Policy
Your first consultation includes your consultation questionnaire assessment and a one-hour session for $125. Subsequent one-hour sessions are $95, and subsequent half-hour sessions are $65.
If you need to cancel or postpone a scheduled session, and you notify me within 24 hours before your session, there will be no charge. If you do not notify me within 24 hours before your session, there will be a $50 cancellation fee.
Privacy and Confidentiality
Your Dynamic Emotional Integration consultation will remain confidential between us. Your information will not be shared, and your contact information will not be used for any other purpose.
However, if we are working electronically (by phone, Skype, Zoom, e-mail, or other electronic formats), it is possible that our privacy could be breached accidentally as a result of technical issues in the transmission of data. It is also possible that our privacy could be breached intentionally through hacking or surveillance activities.
I take precautions to safeguard my electronic communications, but there are privacy risks inherent in electronic communications that I want you to be aware of.
Before you fill out your questionnaire: If you are uncomfortable sharing private information through email or other electronic forms of communication, please contact me and we will discuss alternative methods.
